ABSTRACT

OBJETIVO: Avaliar o impacto que a abertura do sulco interatrial representa em termos de ganho de parede de átrio esquerdo quando da retirada de coração e pulmões para transplante. MÉTODOS: Foi estudada a medida do átrio esquerdo, pela face mediastinal direita, obtida após dissecção do sulco interatrial em 50 cadáveres humanos. O ganho em extensão de parede do átrio esquerdo após a abertura do sulco interatrial foi correlacionado com as variáveis sexo, idade e grupo étnico. RESULTADOS: O ganho obtido através da abertura do sulco interatrial à direita foi, na média, de 1,31 cm (variação de 0,3 cm a 2,5 cm). Quando esse valor foi relacionado à variável sexo, observou-se que nos 27 (54 por cento) casos do sexo masculino o ganho foi de 1,19 cm ± 0,6 cm e, nos 23 (46 por cento) do sexo feminino, o ganho foi de 1,21 cm ± 0,5 cm (p = 0,895). Em relação à idade, observou-se que nos 24 (48 por cento) casos com idade menor do que 50 anos o valor foi de 1,08 cm ± 0,6 cm e, nos 26 (52 por cento) com idade maior ou igual a 50 anos, 1,36 cm ± 0,6 cm (p = 0,088). Em relação ao grupo étnico, observou-se que o valor do ganho nos 31 (62 por cento) casos de brancos foi de 1,34 cm ± 0,5 cm e, nos 19 (38 por cento) de pacientes não-brancos, 1,27 cm ± 0,4 cm (p =0,589). CONCLUSÃO: Embora não haja correlação entre o ganho obtido com o sulco interatrial dissecado e as variáveis sexo, idade e grupo étnico, nossos resultados confirmam que a abertura do sulco interatrial pode propiciar acesso a uma extensão significativa de parede atrial esquerda na separação do coração dos pulmões.


OBJECTIVE: To evaluate the impact of the interatrial sulcus opening regarding of left atrium gain when harvesting heart and lungs for transplantation. METHODS: It was evaluated the left atrium dimension, from the right mediastinal side, after the interatrial sulcus dissection in fifty human cadaver. The interatrial sulcus gain was related with gender, age and ethnic group. RESULTS: The gain observed through right interatrial opening was, in media, 1.31cm (0.3 cm to 2.5cm). When that value was related to the variable gender it was observed that in the 27 (54 percent) cases of the male the earnings was 1.19 cm ± 0.6 cm and, in the 23 (46 percent) female, 1.21 cm ± 0.5 cm (p = 0.895). In relation to age, it was observed that, in the 24 (48 percent) cases with smaller age than 50 years, the value was 1.08 cm +0.6 cm and, in the 26 (52 percent) with larger age or same to 50 years, 1.36 cm ± 0.6cm (p = 0.088). In relation to ethnic group, it was observed that the value of the earnings in the 31 (62 percent) cases of whites it was 1.34 cm ± 0.5 cm and, in the 19 (38 percent) non white, 1.27 cm ± 0.4 cm (p = 0.589). CONCLUSION: Heart grafts and lung grafts must be harvested maintaining adequate segments of left atrium near the heart and the pulmonary veins of the lungs that will be transplanted. Although there is no correlation between the observed gain in the interatrial sulcus dissection and the variables gender, age and ethnic group, our results confirm that the interatrial sulcus opening can give access to significant extension of left atrium wall when separating heart and lungs.

ABSTRACT

Bochdalek´s hernia is a congenital malformation of the posterolateral diaphragm region. It is more common on the left and more frequently seen in newborns and rare in adults, with over a few 100 reported cases. We present a case of Bochdalek´s hernia in a 49-year-old patient with long term dyspeptic symptoms. The upper endoscopy showed a gastric fundus herniation sliding into the chest through the diaphragmatic defect. The patient also presented with a rare pulmonary malformation diagnosed during surgery. It was corrected through thoracic approach with no other complications.

ABSTRACT

PURPOSE: Despite the advances in microvascular free tissue transfer for intraoral reconstruction, this surgery is not recommended for all patients. In specific cases, the pedicled temporoparietal galeal flap may be an option for reconstructive procedures in the head and neck regions. The objective of this paper is to present the anatomical aspects of a galeal flap based on the superficial temporal vessels and to test its potential for reconstructing diverse sites of the oral cavity and pharynx. METHODS: We performed 40 dissections on 34 fresh adult cadavers. The flap vascular anatomy was studied by injecting latex into the superficial temporal vessels. A standardized square-shape flap measuring 10 x 10 cm², pedicled on the superficial temporal vessels, was raised. Oral cavity and oropharynx reconstruction simulations were performed after flap transposition into the mouth by passing it under the zygomatic arch. Hypopharyngeal reconstruction was tested by transposing the flap to the neck under the facial nerve. RESULTS: After latex injection, a rich vascular network over the temporoparietal galea was observed directly from the superficial temporal artery, and a well-vascularized flap based on this vessel was raised. In the reconstruction simulations, the flap was shown to be suitable for the coverage of hypothetical defects in most oral cavity and pharyngeal sites, mainly the retromolar trigone, tonsil area, and buccal mucosa. CONCLUSIONS: A galeal flap based on the superficial temporal vessels presents favorable anatomical characteristics for oral cavity and pharyngeal reconstruction.

ABSTRACT

Groin hernias emerge at the myopectineal orifice of Fruchaud which is closed off by the fascia transversalis. Our previous studies showed structural and quantitative changes of the fascia transversalis elastic fibers of inguinal hernia patients and elderly people. The present study used single-strand conformation polymorphism (SSCP) elastin (analysis to investigate the 34 exons of the ELN gene of 49 inguinal hernia patients (7 females, 42 males aged 58.7 ± 19.82 years) and 75 non-herniated controls (35 females, 40 males aged 46.2 ± 14.32 years). We found that 47 patients and 24 controls had an abnormal exon 20 pattern caused by a g28197A > G missense mutation leading to an S422G amino acid substitution in the elastin hydrophobic domain. The g28197A > G allele frequency was 0.71 ± 0.045 in hernia patients and 0.21 ± 0.030 in controls and 23 patients and 7 controls were g28197A > G homozygous and 24 patients and 17 controls were heterozygous. This point-mutation showed a statistically significant association with inguinal hernia, chi-squared being 46.89 (p < 0.001) and the odds ratio 49.93 (95 percent confidence interval of @11 to 223). These results indicate that the g28197A > G mutation is involved in the genesis of inguinal hernia (possibly due to abnormal elastic fiber production) and explains impaired fascia transversalis function.

ABSTRACT

O implante auditivo de tronco cerebral é uma opção os pacientes surdos que não têm a integridade das vias auditivas preservada. A cirurgia, por sua complexidade anatômica e funcional, requer treinamento específico em laboratório de anatomia por parte do cirurgião. OBJETIVOS: Estudar a anatomia cirúrgica da cirurgia do implante auditivo de tronco cerebral. FORMA DE ESTUDO: Estudo anatômico. MATERIAL E MÉTODO: Neste estudo dissecamos cadáver fresco preparado com solução corante injetada nas artérias e veias intra-cranianas. O local de inserção do eletrodo do implante auditivo de tronco cerebral foi estudado através do acesso translabiríntico. RESULTADOS: A técnica cirúrgica utilizada para a implantação do eletrodo de tronco cerebral é semelhante à utilizada na remoção do shwannoma vestibular. O complexo de núcleo coclear, composto pelo núcleo coclear ventral e dorsal, é o local para a colocação do eletrodo. O núcleo coclear ventral é o principal núcleo de transmissão de impulsos neurais do VIII par e seus axônios formam a principal via ascendente do nervo coclear. Tanto o núcleo ventral como o dorsal não são visíveis durante a cirurgia e sua localização depende de identificação de estruturas anatômicas adjacentes. CONCLUSÃO: A região de implantação do eletrodo do implante auditivo de tronco cerebral apresenta referências anatômicas que permitem sua fácil identificação durante a cirurgia.

ABSTRACT

No presente estudo as alterações etárias no sistema capilar foram investigadas nos músculos diafragma e reto anterior do abdome em ratos de 1, 8 e 18 meses de idade. Os padrões de ativação destes músculos diferem em que o diafragma é regularmente mobilizado muitas vezes a cada minuto durante toda a vida do animal, enquanto o reto anterior do abdome, embora mobilizado na respiração seja ativado de maneira muito menos freqüente e regular. As fibras musculares foram pré-incubadas em pH 4,35 e fixadas pelo método de Padykula e Herman para estudar a atividade miofibrilar da ATP-ase, tornando possível a separação das fibras em três grupos: oxidativa lenta (I); glicolítica oxidativa rápida (IIA) e oxidativa rápida (IIB) e a identificação dos capilares ao redor das fibras no mesmo corte. A relação capilar/fibra e a densidade capilar foram obtidas. A análise da relação capilar/fibra e da área entre os dois tipos musculares mostrou padrões diferentes de desenvolvimento capilar, no envelhecimento, das fibras não fatigáveis (I e IIA) do músculo diafragma em comparação com o reto anterior do abdome. Estas diferenças etárias do diafragma e reto anterior do abdome poderiam estar relacionadas à contínua contração diafragmática e poderia vir a ser este um modelo natural de exercício permanente e envelhecimento muscular.

ABSTRACT

OBJECTIVE: The objective of this study is to evaluate the benefits of drainage in the Stoppa procedure for inguinal repair. PATIENTS AND METHODS: The use of a suction drain was randomized at the end of the surgical intervention in 26 male patients undergoing inguinal hernia repair, divided into 2 groups: Group A, 12 patients undergoing drainage, and group B, 14 patients not undergoing drainage. On the second postoperative day, all patients underwent abdominal pelvic computed tomography scan examination to detect the presence of abdominal fluid collection. RESULTS: In group A, no patient developed fluid collection in the preperitoneal space, and 1 patient presented with an abscess in the preperitoneal space on the 15th postoperative day. In group B, 12 patients presented with fluid collections in the preperitoneal space on computed tomography scan evaluation. However, only 3 patients presented minor complications. None of the patients developed a major complication. CONCLUSION: The use of suction drainage with the Stoppa procedure does not provide any benefit

ABSTRACT

OBJECTIVE: To report a case of bilateral giant renal angiomyolipoma associated with tuberous sclerosis, with successful treatment, and to review the literature concerning angiomyolipoma treatment. CASE REPORT: Patient with tuberous sclerosis and angiomyolipoma diagnosed by ultrasonography during her pregnancy. At that time, the angiomyolipoma on the right side was 9 cm in diameter. Conservative management was selected during her pregnancy. The patient returned 7 years later, with a 24.7 x 19.2 x 10.7 cm tumor on the right side and another of 13 x 11.5 x 6.5 cm on the left side, in addition to multiple small angiomyolipomas. A nephron-sparing surgery with tumoral enucleation was performed on the right side, and after 3 months, the tumor on the left side was removed. Renal function in the post-operative period was preserved, and contrast medium progression was uniform and adequate in both kidneys. CONCLUSION: We conclude that an angiomyolipoma larger than 4 cm should be removed surgically, since they have a greater growth rate and pose a risk of hemorrhage. Resection of smaller tumors is safe and has decreased morbidity. Tumoral enucleation is an effective treatment method that preserves kidney function

PURPOSE: Our previous studies demonstrated structural and quantitative age-related changes of the elastic fibers in transversalis fascia, which may play a role in inguinal hernia formation. To verify whether there were differences in the extracellular matrix between direct and indirect inguinal hernia, we studied the amount of collagen and elastic fibers in the transversalis fascia of 36 male patients with indirect inguinal hernia and 21 with direct inguinal hernia. MATERIAL AND METHODS: Transversalis fascia fragments were obtained during surgical intervention and underwent histological quantitative analysis of collagen by colorimetry and analysis of elastic fibers by histomorphometry. RESULTS: We demonstrated significantly lower amounts of collagen and higher amounts of elastic fibers in transversalis fascia from patients with direct inguinal hernia compared to indirect inguinal hernia patients. The transversalis fascia from direct inguinal hernia patients showed structural changes of the mature and elaunin elastic fibers, which are responsible for elasticity, and lower density of oxytalan elastic fibers, which are responsible for resistance. These changes promoted loss of resiliency of the transversalis fascia. CONCLUSION: These results improve our understanding of the participation of the extracellular matrix in the genesis of direct inguinal hernia, suggesting a relationship with genetic defects of the elastic fiber and collagen synthesis

ABSTRACT

Com o objetivo de comparar os ligamentos cruzados da articulação do joelho com o ligamento patelar, os autores realizaram análise da quantidade de fibras elásticas entre esses ligamentos por meio de amostras obtidas de ete cadáveres. O estudo histomorfométrico do sistema de fibras elásticas revelado por métodos seletivos de coloração mostrou não haver diferença na densidade linear de todo o sistema de fibras elásticas. Não foi observada diferença estatisticamente significante na quantidade de fibras elásticas maduras entre os três ligamentos estudados. Entretanto, a quantidade de fibras elásticas mduras e elaunínicas, fibras responsáveis pela elasticidade, foi maior no ligamento cruzado anterior. Dessas forma podemos inferir que o ligamento cruzado posterior e o ligamento patelar têm mais fibras oxitalânicas, que são responsáveis pela resistência tecidual. Os autores concluiram que o ligamento cruzado posterior e o ligamento patelar têm estrutura semelhante e parecem mais relacionados à transmissão de força, enquanto o ligamento cruzado anterior está relacionado à elasticidade da articulação de joelho.

ABSTRACT

In order to evaluate age related changes of the elastic fiber system in the interfoveolar ligament, we studied the deep inguinal ring from 33 male cadavers aged from stillborn to 76 years. Selective and alternated staining methods for elastic fibers were performed to differentiate oxytalan, elaunin, and mature elastic fibers. We confirmed quantitative changes of the elastic fiber system with aging. There was a significant and progressive reduction of the oxytalan fibers (responsible for tissue resistance) and a significant increment in the mature elastic and elaunin fibers (responsible for tissue elasticity). Furthermore, there were structural changes in the thickness, shortness and curling of these mature elastic fibers. These changes induced loss of the elastic fiber function and loss of the interfoveolar ligament compliance. These factors predispose individuals to the development of indirect inguinal hernias that frequently emerge in adults and aged individuals, especially above the fifth decade

ABSTRACT

O objetivo do presente estudo foi determinar a sensibilidade e a especificidade do exame de ressonância magnética (RM) em detectar alteraçöes na porçäo superior do lábio acetabular (LA). Foram estudadas 10 articulaçöes do quadril direito, obtidas de cadáveres do sexo masculino com idade variando entre 28 e 44 anos. As peças foram revestidas com resina para manter posiçäo anatômica fixa e conhecida. Com o obejtivo de avaliar a sensibilidade e a especificidade do exame de ressôncia magnética (RM)em detectar alteraçöes da porçäo superior do LA, as peças emblocadas foram submetidas ao exame de (RM) com imagens coronais poderadas em T1. Os blocos de resina contendo as peças foram congelados e seccionados em orientaçäo coronal. Estes cortes anatômicos foram submetidos ao exame macroscópico e, quase metade deles ao exame histológico. O LA estava presente em todos os 102 cortes anatômicos intra-articulares, mas o exame de RM detectou a presença do LA em 49 imagens correspondentes.

A demora na adocao da conduta de exploracao cirurgica pode acarretar sequelas graves na eventualidade de ter ocorrido lesoes esofagicas e/ou traqueais, pois a infeccao, propagando-se pelo mediastino, certamente causara o obito. Este trabalho teve por objetivo identificar e analisar os varios parametros que pudessem sugerir o tratamento mais indicado para pacientes com ferimentos penetrantes da regiao cervical. O estudo foi prospectivo, nao randomizado, e compreendeu 53 vitimas de ferimentos penetrantes da regiao cervical atendidas no Servico de Cirurgia e Emergencia do Hospital das Cinicas da Faculdade de Medicina da Universidade de Sao Paulo, no periodo de outubro de 1990 a setembro de 1993...

ABSTRACT

A case of primary extragonadal yolk sac tumor in the retroperitoneum of a young adult male is reported. The symptoms were melena and weakness for two months. Radiologic studies suggested a retroperitoneal tumor infiltrating the duodenum, artery aorta and vein cava, was found. Partial resection was performed, remaining tumor around the vessels. Microscopic examination disclosed a yolk sac tumor infiltrating the duodenum. The patient was managed unsuccessfully with radiotherapy, but good results were actived with chemotherapy. Few cases like that were reported in the literature
